Health Disparities Conference Bus Survey

Hello UTHSC-Memphis Researchers:

Transportation via motor coach will be provided, free of charge, for those from the UTHSC-Memphis campus who are planning to attend the Delta Consortium Health Disparities Conference on Monday, August 19, 2019, in Jackson MS. Due to limited seating, we must have an accurate headcount of those who are committing to riding the bus.

Please complete this survey to reserve your bus seat. Seats will be given on a first respond, first reserved basis.

Specific details regarding day-of transportation will be sent as we get closer to the conference date.

This survey will close once all seats have been claimed. Any UTHSC-Memphis researchers who did not make a bus seat reservation need to provide their own transportation to the conference. If you would like to be placed on the waiting list, please e-mail the administrator, Derita Bran, [email protected]. Names will be taken on a first come, first serve basis.

Please note, being placed on the waiting list does not guarantee you will receive a seat, so please make alternate arrangements. We will notify those on the waiting list if they have been granted a seat at least 24 hours before the conference date.
