Healthcare Habits

Hello!

You are invited to participate in my survey about how students, professors, and adults of other occupations prioritize a visit to the doctor, and how the negative aspects of healthcare, concern for public health, insurance, and other important factors, influence their decisions to seek medical advice. In this survey, approximately 30 people will be asked to complete a survey that asks questions about their healthcare habits. It will take approximately 15 minutes to complete the questionnaire. Your participation in this study is completely voluntary, and it will not affect your position in any organizations or academic institutions or their benefits. There are no foreseeable risks associated with this project. You will not be compensated for your participation. If you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions.

I understand that by opening this survey, I am giving informed consent to be involved in the research stated above. I have been informed and given the opportunity to ask questions. I fully understand the purpose and topic of the research project, along with the credentials of its Investigator, as said in the project description. I understand that I will be cited anonymously, and my personal information will be available to the Investigator of the project, if interviewed, but will be confidential to all other people.
